The CMSA Foundation is pleased to offer a special CCM Exam package in partnership with the Case Management Institute.

This scholarship package will be awarded to three recipients. Each scholarship package is valued at $886.97.

CCM Exam Scholarship Package contains:

1. On Demand CCM Prep Course (Value $397)

2. CCM Certification Made Easy, 3rd Edition (Value $69.97)

3. CCM Exam in August 2024 or December 2024 (Value $420)

CCM Exam Scholarship Qualifications:

• Must be a new applicant to sit for the Certified Case Manager (CCM) exam.

• Must meet the requirements to sit for the CCM Exam according to the Commission for Case Manager Certification.

• Must be available to register and sit for the exam in either August 2024 or December 2024, and meet the required deadlines for application.

• Complete the Scholarship Terms form below by April 1, 2024.

• Make a minimum $25 tax-deductible donation to the CMSA Foundation by April 1, 2024.

  • $25 Donation = 1 entry

  • $50 Donation = 3 entries

  • $150 Donation = 10 entries
