The work of the CMSA Foundation supports the evolution of case management by funding research, supporting public policy advocacy, and furnishing valuable resources for innovation. The compendium of evidence-based practices that the Foundation is building provides examples of successful case management interventions that can be replicated in a diverse array of practice settings. The Foundation library assists case managers and administrators develop effective, financially sustainable programs in a value-based world. Additionally, the library offers sound models for groundbreaking programs that advance health equity using integrated case management techniques. This poster will describe the mission, vision, and work of the CMSA Foundation. It will also profile a past CMSA Process Improvement Award winner and its subsequent expansion of services after receiving recognition from the Foundation. This successful program represents an example of the evolution and expansion of case management initiatives that the Foundation drives.
